Free vs paid clinic management software: what it really costs
A free download still has running costs. Work out the full yearly cost of any clinic management software and know what to ask before you pay or enter a single patient.

Free clinic management software is a sensible place to start when the budget is tight, but the licence is not the only cost. Reminder messages, backups, moving your records and training your team cost money whether the software is free or paid. This guide shows you how to add up the full yearly cost of each option and compare clinic management software with your own numbers.
What free clinic management software actually gives you
"Free" usually means one of five offers, and each has a cost that shows up later:
- A time-limited free trial: the full product for a short period, then a subscription. It is the best way to test software on your real schedule before you pay.
- A free plan with limits: a cap on patients, doctors or messages, and you pay once you pass it. Check the cap against your patient count and monthly messages before you start.
- Open-source software: the licence is free, while the server, security updates, backups and support are yours to run or to pay a technician for.
- Downloadable desktop software: it runs on one computer in the clinic. Ask when it was last updated, and avoid copies from unknown sources, which can bring malware onto the machine that holds your patient files.
- A spreadsheet template: one file for patients, appointments and payments, and a good start for a small clinic. Our free clinic Excel template lets you move your patients into Clinic Tek later without retyping them.
Where your patient data lives in free and desktop software
Desktop software keeps records on the disk of the computer it is installed on. If that disk fails, the computer is stolen or ransomware locks it, the records are gone unless you have a recent backup somewhere else. With desktop software, making that backup is your job.
The operating system matters too. Microsoft ended support for Windows 10 on 14 October 2025, and only PCs enrolled in Extended Security Updates still receive security fixes (Microsoft's Windows 10 end-of-support page). If a free program only runs on an old system, your patient files sit on a computer that no longer gets security patches.
Before you enter a single patient, check three things. Can you export every patient to Excel or CSV in one step? Where are backups stored, and how often are they made? Who can open patient files, and is that access logged? Software that will not export your data keeps it locked in, so rule it out whatever it costs. Our Clinic Tek security and privacy page explains how we protect patient data.
Work out what clinic management software costs per year
Many vendors do not publish prices, so you will need a quote from each. Ask everyone for the same thing so the numbers compare: the yearly price for your whole clinic, with the doctors and staff you need, including messages, support and updates.
Then fill in this table for each option:
| Cost item | Free or desktop software | What to ask a subscription vendor |
|---|---|---|
| Licence or subscription | Nothing, or a one-off payment that may come with a yearly maintenance fee | The yearly price for your clinic, and what changes when you add a doctor or a branch |
| Reminder messages | A separate messaging provider you contract yourself | How many messages a month are included, and what each extra one costs |
| Server and backups | A computer, an external drive and a staff member's time | Where backups are kept, how often they run and how you restore a deleted record |
| Moving your data | Mostly typing it in by hand | Whether it imports an Excel file, who runs the import and whether that costs extra |
| Training and support | Your team's trial-and-error hours | Whether training and support in your language are included |
| Devices | A desktop computer on a supported operating system | Whether it works in a browser and on a phone |
Add the items up for a full year: the subscription, extra messages, training hours multiplied by your staff's hourly cost, backups, and any setup or migration fee. That total is the number to compare, because it is what you will actually pay.

The costs that do not show in the price
WhatsApp messages. Since 1 July 2025, Meta charges businesses for each template message delivered through the WhatsApp Business Platform, at a rate set by the message category and the country code of the recipient's number. Service messages are free, and utility templates such as appointment reminders are also free when delivered inside an open customer service window after a patient's message (WhatsApp Business Platform pricing). Ask each vendor whether reminders are part of the subscription or billed to you per message.
Front-desk time. If your receptionist phones patients to confirm tomorrow's visits, count those hours each month. You pay them with any software that does not send reminders on its own.
Moving your data. If retyping one patient takes one to two minutes, a thousand patients take 17 to 33 hours of data entry. Software that imports an Excel file turns that into one upload.
Training. Ask how long a typical team takes to learn the system and multiply it by your number of staff. A week of slow mornings at the front desk is a real cost.
Yearly subscription or one-off purchase
A one-off purchase suits a clinic that often works offline and has someone to manage its computers and backups. Updates may be sold separately, and anything new, such as WhatsApp reminders, needs another purchase or integration.
A subscription suits a clinic that wants the schedule on more than one device and wants the vendor to handle updates and backups. It is a recurring payment, so total it over three years and compare it with the purchase price plus three years of maintenance and updates.
Decision rule: if nobody in your clinic looks after computers and backups, a subscription carries less risk for you, even when it costs more in year one.
How to compare before you pay
- Write down what you need: the number of doctors and staff, your specialty, and whether you need WhatsApp reminders and insurance billing.
- Ask every vendor for the yearly price for your whole clinic in the same format, and fill in the cost table above.
- Run a real week of your appointments through the trial: book a visit, send a reminder and issue an invoice.
- Export your patient data during the trial to prove you can leave whenever you want.
- Compare the first-year total and the three-year total, and pick the lowest among the systems that passed the trial.
