A no-show and cancellation policy for a clinic or medical office is a written rule patients see before they book: how late they can cancel without any charge, and what happens if they miss a visit without telling you. This guide gives you a policy template to copy, then helps you choose what fits your clinic for each clause.

Why your clinic needs a written no-show policy

No-shows cost more than any single day shows. A systematic review of 105 studies found an average no-show rate of about 23%, and the factors most often linked to no-shows were a long gap between booking and appointment and a history of missed visits (Dantas and colleagues, Health Policy, 2018).

Two rules follow. Confirm the day before any visit booked two weeks or more ahead, and watch patients who have missed visits before. A written policy also means the whole team applies the same rule, so reception does not negotiate with each patient.

A policy template to copy

Replace everything in square brackets and delete any clause you do not need. Keep the notice period shorter than the gap between your reminder and the visit: with a reminder the day before, 12 hours gives every patient time to reply to it. If you ask for longer notice on long sessions, send their reminder two days ahead.

Appointment policy at [clinic name]

We set the doctor's time aside for you alone, so we ask the following:

  1. Cancelling or changing: tell us at least [12] hours before your appointment, by replying to your reminder or calling [clinic phone].
  2. Arriving late: if you are more than [15] minutes late, we may offer you another time so patients after you are not kept waiting.
  3. Missed appointments: after your [second] missed appointment without notice, we ask for [a deposit / a no-show fee] of [amount] when you next book.
  4. Long sessions: we ask for a deposit of [amount] to hold [session type]. It comes off the session invoice, and we refund it in full if you cancel at least [48] hours ahead.
  5. Emergencies: this policy does not apply to medical or family emergencies.

Thank you for helping us see every patient on time.

Before you charge any fee, check the rules that apply to your clinic, and get the patient's written agreement to the policy when they book.

Deposit or no-show fee

A deposit is paid at booking and comes off the visit invoice, so the patient commits before the appointment and you never chase money after a no-show. It suits long sessions and high-value visits such as implants or laser treatment.

A no-show fee is charged after the patient misses the visit. It is harder to collect and can cost you the patient, so only ask for it from patients who agreed to it in writing when they booked.

Decision rule: start with reminders for every appointment, add a deposit for long sessions and keep the fee for repeat no-shows. Read how booking deposits reduce cancellations before you set the amount, then see deposit payment links in Clinic Tek for how a deposit is requested and recorded on the patient's invoice.

How to tell patients about the policy without losing them

  • Mention it in one line at booking, for example "Please let us know 12 hours ahead if you cannot come", and send the full text to anyone who asks.
  • Post it at reception and on your clinic's booking page.
  • Make cancelling easy: one reply to the reminder is enough. A patient who cancels early gives you time to fill the slot.
  • Explain the reason in the patient's terms: the time is held for them, and a missed visit delays someone else who is waiting.

For the reminder wording itself, see our dental appointment reminder message templates; they work for other specialties with small changes.

When to make an exception

Clear exceptions protect your relationship with patients. Agree on these cases with your team in advance:

  • A medical emergency for the patient or a family member.
  • A patient's first no-show: remind them of the policy and waive any charge.
  • The clinic ran late or changed the appointment itself.
  • Something outside anyone's control, such as a storm or a closed road.

Record each exception in the appointment notes, so the team sees the same decision when the patient books again.

Applying the policy automatically

Clinic Tek handles the day-to-day part of the policy:

  • Every patient who has not opted out of WhatsApp messages gets a reminder the day before, with buttons to confirm, cancel or reschedule, and the answer shows on the schedule straight away. See WhatsApp appointment reminders for clinics.
  • When a patient cancels from the reminder, the appointment is cancelled on the schedule, so you can give the time to someone else.
  • You mark a missed visit as a no-show, and reports show your no-show rate for each period, so you can see whether the policy works.
